Delilah Hernandez
Patient Care Coordinator
Originally from Miami, Florida, she spent her early years there before moving to Hartford, Connecticut, where she completed high school. Her passion for helping others began through her involvement with a youth corps and a variety of community-focused programs. She participated in city hall events, food and clothing drives, community projects, and volunteer opportunities, all of which helped strengthen her desire to serve and support others.
Since graduating in 2024, she has worked exclusively in orthopedic medical offices, gaining valuable experience in several areas of patient care and office operations. Her background includes working alongside therapy departments, supporting patients at the front desk, and communicating with billing teams to help ensure a smooth and positive patient experience. This well-rounded experience has given her a strong understanding of both the clinical and administrative sides of a medical office.
Outside of work, she enjoys traveling, modeling, going to the gym, practicing lash artistry, creating content on TikTok, and continuing to learn about day trading.
She describes herself as kind, understanding, welcoming, and outgoing. She wants every patient to feel comfortable, supported, and heard from the moment they walk through the door. She is always happy to listen, answer questions, and assist patients in any way she can throughout their experience at Hohman Rehab.
